Our employee had an incident but does not want to seek medical treatment, do we need to send in a claim?

Accepted Answer

Yes, but you MUST have the employee sign the Refusal of Medical Treatment form (available in the Cornerstone PEO App or in your Client Packet), then simply report the claim as Report Only and be sure to include the Refusal of Medical Treatment form. The ensures the claim is then processed as a “Report Only” claim.

In most states the injured worker has 30 days to change their mind after signing the Refusal of Medical Treatment form. If the injured worker decides to seek medical treatment during that time, you already have the claim filed so no late charges or penalties will be incurred.

I don’t remember my password. How can I login?

Accepted Answer

You can go to our website at https://cornerstone.prismhcm.com then click on Reset Password, then enter your email address and then click in the box to the left of I’m not a robot. Then enter your email address and hit enter.  The system will send you an email to your email address on file with Cornerstone PEO.  The email will come from info@prismhcm.com and will have a link that you can click on to reset your password.

Does the injured employee have to seek medical treatment to return to work after an injury?

Accepted Answer

No, if no medical treatment is needed, or the employee wishes to seek medical treatment under their own insurance with their primary treating physician, they must sign the Refusal of Medical Treatment form included in the Claims Packet and available on the Cornerstone PEO App.

Accepted Answer

If you get the message that your account is locked, you will need to contact us at 877-736-9473 and choose Option 0. You will need to answer a few questions to identify yourself.  Once verified, we can unlock your account so you can access the system.

No, if no medical treatment is needed or the employee wishes to seek medical treatment under their own insurance with their primary treating physician, they can sign the refusal of medical treatment included in the Claims Packet.

Before you can login to our payroll software, there are a few things that must happen first.  You must fully complete your electronic onboarding, and your worksite employer must login and approved you.

You will need the alphanumeric code provided in the Electronic Employee Onboarding letter from your employer. The code is highlighted in the upper right corner of the letter. It is composed of 4 numbers followed by two letters. The injured employee wants to seek medical treatment, where do I send them?

If it is a medical emergency, call 911 or take them to the nearest emergency room.

For non-emergent CA claims, please click on the following link to find a network provider in your area.

For non-emergent CO, GA, PA, VA or TN claims, you will need to provide a panel list of doctors to the injured worker if you do not have one already displayed at your business or provided in your initial employment packet. To find a panel list of doctors please click the following link, scroll down and click the Provider Panels button. From there, choose the state and area that you need and a panel list of doctors will be generated.

For all other states, you may go to any urgent care facility that takes workers’ compensation.

The injured employee wants to seek medical treatment, where do I send them?

You will need your 4-digit company code as the username and your password, which was provided in the onboarding email you received. If you have been in the system before and forgot your password, you can reset your password, and an email will be sent the address associated with this account.

Please go to https://www.cornerstonepeo.com/w2-request-form to complete and submit a W2 correction form. A team member will be in touch with any questions and corrections that were made.

These types are issues can be tricky under workers compensation. If the employee fell, the injuries from the fall would likely be compensable.  If the issue was caused by something directly involving their work environment, it may be compensable. If in doubt, report the claim or call us at (640) 529-6124.

Taxes are deducted based on your gross (overall) wages in a paycheck and the number and kind of dependents you have recorded on your W4 tax form. You can update your W4 to adjust Federal Taxes. Then log in using your username and password, then go to Federal Tax Forms, then click on W4, then click on Update W4, make the change, then click on Save at the bottom of the screen.

All premiums are deducted from the employee's paycheck automatically once they enroll in our Benefits Management System. No need to worry about elections or managing deductions!

Yes, send in a claim but have them sign the refusal of medical treatment form included in the claims kit. The claim is then processed as a report only claim and no claims fees are charged.

You will need your login information to make a change. Then log in using your username and password, then go to Personal, then Direct Deposit, then Edit to change your Direct Deposit or New to add an additional direct deposit. Then click on Save. If you have more than one direct deposit, the one without a specific amount or percentage should be set to Remainder of Check.

The days you worked may fall into different pay periods so they would not be in the same paycheck. Please contact your worksite employer for confirmation on the pay period dates for the dates in question that are not in your paycheck.

Any discrepancies on hours or pay rates need to be reported directly to your Worksite Employer, as Cornerstone only enters the payroll information they are given by the Worksite Employer. Cornerstone usually does not have access to the scheduling site used by your company. Please reach out to your contact at your company.
